Bariatric surgery is meant to assist individuals who are struggling with obesity attain a healthy weight and lifestyle.

Millburn Surgical Associates will consider surgical weight loss procedures such as laparoscopic gastric bypass or Lap Band surgery once other non-surgical techniques have been attempted. Laparoscopic weight loss surgery, performed by making small incisions, means less discomfort, a faster recovery time and the potential for life changing results.
For patients considering weight loss surgery, it is important to know it can be a lengthy process. Each insurance company has a different set of rules for approving the surgery. The staff at Millburn Surgical would be happy to help you understand your insurance policy requirements and will obtain the approval for weight loss surgery. Some of the preliminary requirements may be:
1) From the primary care physician
- A letter stating the diets you have tried & failed including office notes with dates and weights for 6 months or more.
- Any documentation from any sources such as Weight Watchers. Nutritionists, etc
- Blood tests
- Lipid profile
- Thyroid Function Study
- Liver Function
- Cortisol Levels
2) Psychology
- Evaluation & Clearance letter
3) Nutritionist
- Consultation & Evaluation letter
Following an initial consultation with Dr. Kopelan, at least one additional pre-surgical visit will be required. Even if the 3 steps above have been completed, Millburn Surgical Associates cannot guarantee insurance approval. Millburn Surgical will require additional medical testing and clearance prior to performing surgery.
